Where to send your request at each of the 14 NHS boards in Scotland, so you do not have to go hunting.

Use this with Guide: request a deceased relative's medical records and Guide: request your own data. Find your board below, send your letter or form to the contact shown, and keep a copy.

Please check before you post. These details were gathered from each board's own website in June 2026. Boards do change their contact details and forms from time to time. Each board's official page is linked, so if anything looks out of date, open the link or phone the hospital switchboard and ask for the Health Records, Information Governance, or Legal department. A wrong address only causes delay, not refusal.

If your relative was treated by a national service

Some care is provided by national or special boards rather than a local one, for example the State Hospital, the Golden Jubilee, or the Scottish Ambulance Service. If that applies, search for that body's name with the words "access to health records" or phone it and ask for the Health Records or Information Governance team. The same law and the same letter apply.
